$150 for Historical Society members

Registration is not complete until all forms are completed and submitted. Some forms must be completed by your child's physician.

IMPORTANT:
If the Health Assessment Record indicates that your child has allergies or asthma, you must submit an Action Plan written by your child's physician.
If the Action Plan indicates that a medication be administered as part of the plan, an Authorization for the Administration of Medication Form is required AND the medication in its original packaging must be provided by the parent/guardian.
